The Guest Assistance Program (GAP) serves as an ambassador to the entire Department of Social Work and to UMHS patients and families in general. GAP social workers assume leadership for educating, coordinating and assisting patients and families with identifying appropriate resources for environmental challenges related to health care. The program also provides information, education, and support to social work colleagues and medical teams regarding available assistance opportunities as needed. The outcome of these services is to support patients and families, to help maximize their participation in and benefit from health care, and to assist in removing barriers to continuity of care.
Conducting targeted psychosocial assessments to identify and assist with environmental challenges related to medical condition, treatment, hospitalization, and/or readiness to engage in care
Identifying barriers to care that interfere with obtaining maximum benefit from health care and/or maintenance of health, such as transportation, prescription costs, lodging, employment concerns, medical equipment, housing, utilities, literacy, and other financial or social service needs
Determining benefits and financial assistance eligibility and/or available resources, with specialized skill in working with the un/underinsured
Providing appropriate interventions to match patient/family need, including patient education, support, advocacy, and care coordination
Serving as a clearinghouse for financial, insurance, and community resource information for staff, patients and families, and outside agencies
Providing referral and linkage to appropriate community resources
Bachelor's degree in social work from a school accredited by the Council of Social Work Education (CSWE) is necessary. Must have LLBSW or LBSW license.
Excellent interpersonal and written communication skills are critical for success in this environment. Word, Access, Excel and Epic are utilized on a daily basis. This position requires brief psychosocial assessment skills to evaluate eligibility for tangible resources, as well as ability to evaluate needs for referral and intervention for patients and families. This position demands the ability to work well with a diverse population in need of a variety of support and services.
The schedule for this position will be Wednesday-Sunday, 8-hour day shifts. On weekends, this role will be the sole representative of the GAP, therefore candidates need to be comfortable with fast-paced and independent critical thinking, problem-solving, prioritization and management of workload.
Knowledge of local and statewide community resources is desirable, as well as experience navigating complex systems. Previous experience in social work and/or healthcare or social service agencies will be particularly helpful for success in this role, as will advanced interpersonal skill and the ability to establish patient relationships quickly.
