The Oncology Nurse Navigator is a professional nurse who functions as a clinical resource to the Oncology Program, coordinates referrals from physicians and health care facilities for patients, guides patients through the health care system by assisting them with access issues and provides a significant level of patient education related to oncology diagnosis and planned treatment.
ESSENTIAL FUNCTIONS AND DUTIES:
Initiate communication with patient upon learning they have an abnormal/suspicious radiology exam or positive biopsy
Assess and document the need and concern of patient and family individualizing care
Guides oncology patients through the health care system and help patients arrive at scheduled appointments on time and prepared.
Inform patients and primary care physicians of test results
Assist patients in access to services and programs throughout the cancer care continuum
