We are currently expanding our team in Tennessee, and we are looking for a Psychiatric Nurse Practitioner to bring compassion, energy, and focus to their daily work. The Psychiatric Nurse Practitioner will assess and treat individuals with behavioral health conditions and serve as a consultant to a multidisciplinary care team providing integrated medical, behavioral, and psychosocial services. He / she will consult with Advance Practice Registered Nurses (APRNs), Registered Nurses (RNs), and Social Workers on a weekly basis to review assessment findings, provide treatment suggestions to improve outcomes, and support in-home crisis stabilization and family support services. Primary Responsibilities: Direct evaluation of individuals with mental/behavioral health conditions who are identified / referred by the individual's case manager and/or primary care provider, have been hospitalized for medical / behavioral health issues, are at risk of nursing home placement, or not improving with current treatment protocols Provide consultation and treatment recommendations for individuals in the medical setting who experience significant behavioral health issues and may need more intensive or specialized mental health care Participate in collaborative multidisciplinary team meetings to optimize clinical integration, improve efficiency / effectiveness of service delivery, and make recommendations specific to medication adjustment or service implementation Be available to provide occasional ad hoc telephone consultation to medical providers, case managers, social workers, APRNs and RNs for complex / challenging individuals or situations Provide education and information on mental / behavioral health conditions, illness trajectory, and treatment options to staff, patients, families, caregivers, and providers Provide input to organizational quality initiatives
Required Qualifications: Graduate of an accredited Master Level Nursing Program (MSN) Board Certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC) 2+ years of post-graduate experience working as a Nurse Practitioner in a mental health setting Current unrestricted nursing license in good standing in Tennessee Current DEA licensure and Prescriptive Authority in state where applicable Ability to work independently with diverse care teams in a variety of settings Effective organizational, time management and verbal/written communication skills Strong computer skills and experience using an Electronic Medical Record Must have access to high speed internet for home office set up Access to reliable transportation and willingness to travel up to 50% for face to face meetings throughout the state of Tennessee Preferred Qualifications:Certification from the American Nurses Credentialing Center as a Psychiatric Mental Health Nurse Practitioner Experience in gerontology or internal medicine RN / NP experience working in home health, home care setting Experience working with Medicaid or Medicare population Experience leading or supervising clinical staff Experience with eClinicalWorks (eCW) Detail oriented Dependable and reliableCareers at TeamMD: Imagine.
