RESPONSIBILITIES: Under direction, this position supervises the daily activities of the Care Management staff, coordinates and implements effective and efficient Care Management processes which includes Complex Care Management, Children’s Care Management, Medical Social Work and care coordination and performs other duties as assigned.
YOU WILL BRING: • Thorough knowledge of and proficiency in applying effective people management skills, including leading team building exercises, facilitating efficient and effective meetings and/or projects, problem solving, negotiating and influencing • Thorough knowledge of/proficiency with Windows based PC system and Microsoft Word, Excel, Outlook, PowerPoint • Knowledge of: - Standard office business practices, and the ability to operate general office equipment - Health care industry, Medi-Cal program, and managed care - Medi-Cal and related policy - Case management principles and activities - Research, analysis and reporting methods • Ability to: - Effectively supervise and coordinate the work of a workgroup engaged in Care Management activities - Motivate staff and promote an atmosphere of teamwork and cooperation - Collect, interpret and evaluate data; and, to project consequences of recommendations - Work effectively independently, with minimal supervision - Maintain a professional appearance, pleasant and friendly manner - Pay close attention to detail • Education and Experience: - Licensed Clinical Social Worker (LCSW) with current unrestricted CA licensure; - Minimum of five (5) years of experience in a client or patient care setting; - At least one (1) year of case management experience; and - Two (2) years of experience as a full-time supervisor; or an equivalent combination of education and experience that would provide the required knowledge, skills and abilities may be qualifying
