Primary Responsibilities:Provide ongoing training and coaching to Diabetes RN Case Managers Provide patient education to assist with self - management Consult with Medical Director, Pharmacists, Social Workers and other internal teams Identify gaps or barriers in treatment plans to close STARS gaps
Required Qualifications:Active, unrestricted RN licensure in your state of residence Certified Diabetes Educator (CDE) 3+ years of Diabetes experience Proficient level of experience with Microsoft Word and Excel, and the ability to navigate a Windows environment Dedicated, distraction - free work space for home office and access to high speed internetPreferred Qualifications:Previous Managed Care experience Certified Case Manager (CCM) Experience working with the Medicare population Ability to perform in a fast paced, high demand environment Adapts well to frequent change in processCareers with Optum.
