Responsible for the patient care in the Wound Care Center. This includes, but is not limited to, utilization of skills in organization, planning, implementing, evaluating, and providing care through the use of hospital and nursing standards. The Wound Care Specialist is accountable for the oversight of the clinical practice, outcomes and success of the Wound Care Center.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Provides case management to a group of patients providing specific assessments according to protocols and procedures, clinical competence in procedural skills, treatments and patient/caregiver education related to wound care management and acts as patient advocate in delivery and coordination of patient care.
Provides efficient and effective care with the guidance of the physician during the clinic sessions, attends the clinics where scheduled and is dependable. Able to manage a number of patients at the same time with high energy and organization.
Collaborates with other health care providers, Wound Care center Physicians, Program Director and Medical Director, regarding clinic and patient needs Develops age appropriate plan of care in collaboration with patient, family and health care team
May provide case management duties as required Demonstrates operational knowledge of equipment.
Ability to access, manage and update patient medical records through Hospital Electronic Medical Record Information System.
Maintains awareness of comfort and safety needs of patient.
Follows policies and procedures for infection control and facility safety in accordance with regulatory guidelines.
EDUCATION and/or EXPERIENCE Bachelorâ™s degree from a four-year college or university preferred Minimum of one to two years related experience and/or training; or equivalent combination of education and experience required.
PROFESSIONAL EXPERIENCE/BACKGROUND 1-2 years of wound care experience preferred.
CERTIFICATES, LICENSES, REGISTRATIONS Current Montana RN, OT or PT license required upon hire Wound Care Certification preferred. Current BLS required upon hire
