Rutgers, The State University of New Jersey, is seeking a Social Worker I for the Department of Social Work Services for Rutgers Cancer Institute of New Jersey. The primary purpose of the Social Worker I is to provide well-integrated social work services to adult patient systems from age 18 years and older, and facilitate adaptation to a cancer diagnosis. Works toward resolution of social and environmental problems, which interfere with patients and their families obtaining maximum benefits from medical care. Provides consultation to interdisciplinary staff concerning psychosocial factors related to health, hospitalization and disability.
Among the key duties of this position are the following:
Provides initial psychosocial assessment to all Cancer Institute of New Jersey patients and their support systems. This will include the interviewing of patients and their support systems, along with information obtained from patient care staff and/or medical records.
Provides information and referral to appropriate Cancer Institute of New Jersey/RWJUH/SPUH/RWJMS and community resources.
Formulates psychosocial treatment plans. This may include, but is not limited to, assistance with: advanced directives, decision making regarding treatment options, patient/family education, discharge planning.
Provides direct social work services in accordance with social work assessment, diagnosis and treatment. This includes, but is not limited to, individual, marital and family counseling, as well as education and referral to appropriate resources.
Provides case management functions as it pertains to social work responsibilities in relationship to partner institutions.
Promotes cost containment and proper utilization of health care resources through early identification, screening of high risk patients and intervention.
Identifies lack of services for patients and assumes responsibility for developing new programs to enhance patient care; identifies gap in service delivery and develops alternative strategies and/or programs to meet patient needs. This will include development of appropriate support groups.
Minimum Education and Experience:
Master of Social Work (MSW) from a program accredited by the Council on Social Work Education, LSW or LCSW.
Credential Required: New Jersey Professional Licensed Clinical Social Worker or Licensed Clinical Social Worker.
Required Knowledge, Skills, and Abilities:
Must be computer literate with working knowledge of Microsoft Word, Access and Excel.
Demonstrated excellent oral and written communication skills, and the ability to interact with diverse patients, physicians, management, and community members.
Must be able to function under rapidly changing and stressful conditions.
Three (3) years of experience in health care, primarily psychosocial oncology.
