Under supervision of the Branch/ Manager, plans, coordinates and staffs day-to-day work activities of the Infusion nurses and coordinates all infusion-related care. May also perform patient/family teaching and hospital/physician customer service activities. Acts as positive professional representative of SSM Health at Home, promoting collaborative relationships for mutual benefit of internal and external customers. Assists in developing strategic plans and department policies and procedures. Implements patient care actions to meet goals, and monitors adherence to quality assurance, safety and risk management policies. Participates in the interview process and hiring decisions for infusion, as well as trains and assigns work to staff. Local daily travel between branch office, hospitals and physician offices is expected.
Oversees Infusion patient care for an assigned group of clinicians, directing utilization of resources to ensure effective, efficient, appropriate care, safety and patient satisfaction. Oversees case management for a group of patients including review of admissions, assessments and discharges with ongoing monitoring of the plan of care. Communicates with physician offices, pharmacist and other departments to ensure physician orders and lab results are reported and recorded accurately. Performs all duties of a field staff nurse as assigned according to Job description(s) of Infusion Staff Nurse, Pediatric Infusion Staff Nurse and/or Staff Nurse. Provides ongoing education and support to clinicians regarding documentation, case management, resource utilization and quality clinical practice specifically in the area of Infusion nursing of adult and pediatric patients.
Develops and maintains relationships with referral sources and other providers, and functions as clinical liaison between branch operations, pharmacy, hospital staff, physicians and other customers. Utilizes clinical skills to assist referral sources with identification of home IV candidates. Recognizes opportunities for expanding the business relationship with the customers and involves leaders including Vice President of Marketing/Business Development and Branch Manager at the appropriate time to present and negotiate SSM's other programs. Exhibits awareness of SSM's other programs including quality assurance, contractual agreements, reimbursement policies, infusion center connections, etc. Promote new services, therapies and products to referral sources.
Exhibits and maintains knowledge of Infusion nursing, pharmacy services, payment practices and overall understanding of Infusion business.
Provides follow-up and support to other branch departments regarding authorizations, service batch and incomplete and/or missing documentation. Interacts with Infusion Pharmacy staff and other nursing teams to coordinate care seamlessly for patients and families.
Assures appropriate levels of service and supplies are utilized to maximize potential for positive clinical outcomes, while managing resources efficiently and using specialized staff such as dieticians effectively to reach optimal clinical outcomes efficiently.
Reviews cases and performs documentation audits to assure that the Plan of Care is adhered to and effective patient teaching, medication reconciliation and disease management guidelines are utilized effectively. Monitors staff schedules on a daily basis to maintain productivity and quality patient care. Triages patients appropriately.
Participates with Branch Manager in hiring, educating, counseling and evaluating field staff, including field observations of clinical staff to ensure quality of care and client satisfaction. Conducts team meetings and staff meetings monthly and as needed. May provide education to other nursing teams specifically about Infusion nursing.
Provides ongoing education and support to clinicians regarding documentation, case management, resource utilization and quality clinical practice specifically in the area of Infusion nursing of adult and pediatric patients. Participates in coaching and positive staff education regarding Infusion nursing and related topics as assigned.
Monitors key indicators and department operations, making adjustments and recommendations to the Branch Manager to ensure that goals are met, effectively using pending patient, case manager, visit timeliness and LUPA reports. Works with clinicians to improve clinical outcomes and patient satisfaction utilizing clinical data software. In collaboration with other departments, develop and implement actions in the Infusion market plan. Prepare reports as required and requested. Review and analyze referral/admission data to determine strategies and assists in the development of new services to meet market needs.
Provides a personal example of living the principles of continuous quality improvement whereby decision making is driven by data, and customer-supplier relationships are facilitated within the System.

RN Associate's Degree required
Must maintain a valid driver's license and a driver's record deemed acceptable by SSM.
Current Missouri RN license
1 years of RN nursing experience
CPR certification required
Infusion nursing experience required
Customer service focus and skills required
Must maintain and be able to produce proof of current auto insurance with at least state required minimum coverage.
This position requires daily travel to patient homes and meeting locations, so the employee must have demonstrated ability to operate a vehicle in a safe and lawful manner.
Additionally, access to a vehicle in good working condition required.
Requires effective oral and written communication skills to communicate with customers.
Requires good decision making and problem solving skills.
Requires focus on customer service and patient experience including ability to interact with newborn, pediatric and adult patients and families as well as nurses, physicians, pharmacists and other caregivers, including internal customers in SSM entities and departments.
