Provides case management coordination services to include assessment, planning, monitoring, advocacy and direct intervention and provides advisory and consultative services relating to the assigned program area.
35% - Non-Essential - Develops: Develops, coordinates, and implements an individualized plan for service and treatment of juveniles assigned to the center. Coordinates and/or provides training to facility staff on service provisions. The HSP works with the site director on interface with the Department of Juvenile Justice on matters relating to treatment. Prepare the Service Plan for each resident on his/ her caseload, prepare the Service Plan Review, prepare the Discharge Plan while the resident is at Camp at the Discharge Summary after the resident has been discharged. This will be completed in accordance with state regulations.
30% - Essential - Counsels: Counsels with juveniles on an individual and group basis. Focusing on the plan for services in order to meet their needs and to exchange their ability to function in more appropriate ways. Also, counsels juveniles on behavior and progress in all phases of treatment and prepares them to return home or to a less restrictive environment
20% - Essential - Meetings: Attends staff meetings, conferences, and trainings. Is available to parents and other agencies for consultation. Provides family therapy as needed. Coordinates and approves off-camppus activities. Serves as part of the management team and supports all aspects of facility operations to include, but not limited to, security procedures, juvenile assessments, recreation maintenance and staff functions. Participates in activities which enhance professional growth and development.
15% - Essential - On-Call & Other: Serves as a Social Worker on call. Completes other duties as assigned by the Lead Clinical Staff or Site Director.
A high school diploma and relevant program experience. A bachelor's degree may be submitted for the required program experience.
