General Summary: Under the supervision of the PI and the Study Director, the Patient Navigator (PN) will help wrap up PROMOVER, a study conducted to increase linkage and retention in care for Mexican HIV infected persons. The PN will engage and support HIV-positive individuals referred to health care services, and clients who are lost to care. The PN will develop professional relationships with patients receiving HIV primary care at the CORE Center and will assist patients to facilitate culturally and linguistically appropriate linkage and retention in care. The PN will provide mental health support and psychoeducational training.
Principle Duties and Responsibilities:
Establish rapport and build an ongoing effective and positive working relationship with study participants.
Provide adherence and support services including charlas to study participants and assist in identifying and addressing barriers to accessing care.
Encourage participants to identify personal strengths, abilities, and any assets to assist in achieving health goals.
Navigate health systems and appropriate social service agencies with participants to optimize care.
Attend staff meetings and training teleconferences to maintain the mission of PROMOVER.
Establish and maintain working relationships with internal services and personnel including providers, other care team members, data consultant and Evaluator.
Provide financial incentives to participants who have completed study activities.
Record and maintain confidential patient information in a timely and accurate manner.
Conduct follow up activities (including phone-calls, letters, and in person reminders) to ensure that participants are retained in study for follow up visits.
Locate and re-engage participants who would otherwise be lost to care including conducting field outreach.
Help with data collection, report writing and dissemination of findings.
Qualifications: MSW, MPH or MA or Master’s degree in related field. Requirements: Experience using Motivational Interviewing techniques, case management, patient navigation, or medical care coordination. Experience conducting field outreach work for medical or social research studies, specifically, in retaining difficult to engage participants in care or research studies. Experience working with diverse populations affected by HIV/AIDS in clinical and community settings with basic knowledge of related medical terminology. Experience in real-time data entry to web-based and/or software-based systems.
Required: Bilingual English/Spanish with excellent interpersonal skills and non-judgmental approach to engaging clients. Proficiency in Microsoft Word, Excel and data entry experience Familiarity with communities in which study participants reside preferred. MUST complete training in designated human subjects protection with certification by end of first month
