You'll have a direct impact on the lives of our members as you recommend and manage the appropriate level of care throughout the entire treatment plan, via coordination and collaboration with multiple internal and external partners including consumers and their families/caregivers, medical, behavioral health and clinical network teams. Assess and Triage Care Advocate/ Field Care Based work with complex and high-risk needs with a goal of engaging the consumer in the treatment process, decreasing their reliance on higher levels of care, helping them to access appropriate community services, and assisting them in improving community tenure. And, when you contribute, you'll open doors for yourself that simply do not exist in any other organization, anywhere. ***This position requires onsite responsibilities in Phoenix area facilities*** Primary Responsibilities: Focus on initial inpatient admission for psychiatric and chemical dependency patients Utilize advanced clinical skills to engage and motivate Consumers via a recovery, health and wellness-oriented approachMeet with consumers in-person at facilities, as appropriateMake patient assessments and determining appropriate levels of care Collaborate with providers to determine acuity of mental health and substance use disorder concerns, barriers and progress; coordinate appropriate servicesAdminister benefits and assist in the development of appropriate consumer specific treatment plansCoordinate benefits and transitions between various areas of care Assist consumers and their families/caregivers with connections to appropriate psychiatric, medical, and psychosocial referrals and servicesIdentify and remove barriers to procurement, delivery, participation in and success of servicesPartner with designated external providers, programs, entities to address the needs, gaps in care, and recovery goals of complex and high risk consumersEstablish a cooperative relationship with Optum Behavioral Network Services and the Provider community in order to improve access to services and ensure that consumers are served within proper standards of care
Required Qualifications:Licensed Master's degree in Psychology, Social Work, Counseling or Marriage or Family Counseling, or Licensed Ph.D., or an RN with 2 or more years of experience in behavioral healthLicenses must be active and unrestricted in your state of residence2+ years experience in a related mental health environment Proficient Microsoft skills (Word, Excel, Outlook) with the ability to use multiple screens and type and talk simultaneouslyReliable transportation and the ability to travel as needed within the designated service delivery areaCare Advocate will be expected to be onsite at Phoenix area facilities > 75% of the time, but will be able to work from home when not onsitePreferred Qualifications:Experience in an inpatient settingDual diagnosis experience with mental health and substance use disordersStrong preference for candidates with extensive substance use disorder background. Experience working in an environment that required coordination of benefits and utilization of multiple groups and resources for patients Previous experience in a managed care environmentBilingual skills (fluency in Spanish/English)Careers with Optum.
