Northwest Kidney Centers of Seattle has an immediate opening for a Social Worker at its south end clinics (Auburn and Enumclaw). This position will work 8 am - 8 pm, flexing schedule accordingly as prearranged with patients (so, may have early start time or a later start time, depending on social worker/patient agreement) . Position works Monday through Friday.
This social worker joins our great team of social workers who assist patients and their families at our kidney dialysis treatment clinics. The position will be responsible for two of our clinics, the Auburn and Enumclaw centers (south of Seattle).
The Social Worker is responsible to the Social Services Manager for social work, intake and patient advocacy service and for coordinating social services matters with physicians, NKC personnel, patients, families, and other organizations. This position is also responsible for patient education and counseling, and for performing relief and/or project duties, when required.
The Social Worker is part of the interdisciplinary team (IDT), and is responsible for providing each patient with an individualized comprehensive assessment of his or her social service needs, and developing a plan of care in accord with Conditions of Coverage. The Social Worker evaluates the patient’s status, and uses the comprehensive assessment to develop and implement the patient’s treatment plan and appropriate quality care.
The Social Worker is also a part of the Quality Assessment Performance Improvement Team, (QA/PI), and participates in evaluating trends, making recommendations, and following through to achieve improved patient care.
MSW required. Current Washington state Social Worker License required or an ability to obtain one. Previous medical social work experience highly desirable but not required. New graduates will be considered! Valid Driver’s license and proof of insurance required since some local travel is required. Detail-oriented important due to records/charting requirements of position.
