The Montefiore Einstein Center for Transplantation is one of the most active and successful programs in the country, performing adult and pediatric heart, kidney, liver, and pancreas transplants. Our approach unites the outstanding clinical staff at Montefiore Medical Center with the world-renowned basic science faculty at Albert Einstein College of Medicine. This enables us to offer innovative treatments and access to advanced clinical research trials.
The transplant clinical pharmacist holds an integral role within the inpatient and outpatient transplant multidisciplinary teams. This challenging role calls for the clinician to be responsible for effectively enhancing the drug therapy outcomes of transplant patients at the Medical Center. This is accomplished through continual assessment of the appropriateness, safety and efficacy of individualized transplant patient drug therapy in the pre- and post-transplant period. The clinician provides recommendations, information and education to medical/surgical, nursing, pharmacy personnel, and other health care professionals, as well as actively participates in clinical research in transplantation or other related disciplines. Additionally, the practitioner is eligible for faculty appointment at the Albert Einstein College of Medicine and the University of Buffalo School of Pharmacy.
The Solid Organ Transplant Clinical Pharmacy Manager must have advanced training, including completion of a PGY-2 residency in solid organ transplantation (or equivalent 3-5 years of clinical experience), and must be eligible for licensure in the state of New York. Board certification is encouraged and supported by the Department of Pharmacy.
