We are hiring for a Full-time Youth and Family Specialist to join our Calumet Team!
The Youth and Family Specialist completes the assessment process, service planning, and supports to maximize potential of clients to achieve goals and become self sufficient.
Essential Job Functions:
Primarily responsible for coordinating the delivery of services.
Completes the Adult and Youth Intakes to monitor goals and activities.
Provides ongoing engagement, reassessment and goal planning.
Provides support services, referrals, and follow up to meet client needs.
Addresses housing related issues including lease compliance referrals from Property Management.
Coordinates youth activities with CHA’s Resident Services Division, including outreach and coordinating service delivery, including on-site youth programming.
Coordinates events and activities within the region to meet client needs and engage individuals and households in services.
Handles emergency and crisis situations with residents.
Works within contract, grant, and program requirements including meeting timelines and reporting requirements.
Utilizes a team approach and works with other program staff in the planning and implementation of services.
With supervisory approval, authorizes limited amounts of financial assistance to clients.
Takes increasing responsibility for own professional development, utilizing supervision and other developmental opportunities within and outside the agency to enhance skills and understanding.
Other duties as assigned by supervisor.
Works effectively with diverse staff and service populations. Accountable for providing exception customer service.
Tracks youth and family data.
Required to work one Saturday and at least one late night a month.
High School Diploma and five (5) years of experience required.
OR Bachelor’s Degree in Social Work or related field preferred.
Minimum of one (1) year of experience in related field.
Demonstrated ability to render adequate service to clients and to organize and manage job efficiently.
Must have access to a vehicle, valid driver’s license and proof of insurance to perform essential job functions.
Must be highly organized and have adequate knowledge if Microsoft Excel to track data.
Ability to work with diverse individuals and groups.
Bachelor's Degree in Social Work or related field from an accredited college or university or HS Diploma plus five years of case management experience which includes the coordination of youth and family services.
