The Division of General Medicine and Geriatrics is actively building an integrated team practice model for delivering high quality medical care and service to the patients we serve, including transitional care of hospitalized patients. The practice also includes innovative models for delivering structured care to populations of patients with chronic conditions. The Division of General Medicine and Geriatrics is recognized by the State of Oregon as a Tier 3 Patient Centered Primary Care Home and a Comprehensive Primary Care Plus clinic site.

Functions/Duties of Position
The Clinician Educator will work as a primary care clinician, same day or urgent care, in the Faculty Practice 7 half days a week. They will also participate in clinical based teaching as well as potentially in an inpatient general medicine teaching attending role. This position will include both a continuity ambulatory practice and coverage clinic and is designed to provide increased after-hours appointment access for practice's patients as well as increase our overall patient access. Faculty in the Division of General Internal Medicine & Geriatrics participate actively in teaching medical students, residents, fellows, and each other. Opportunities for teaching abound and participation is encouraged and valued. The faculty member will be expected to participate in continuing education, professional and faculty development and scholarship, as directed by his or her department.
MD or DO
Demonstrated ability to work with a team of health care providers and collaborate to achieve clinical care goals
Excellent communication skills and the ability to collaborate with other clinicians
Able to perform the essential functions of the position with or without accommodation
Interest in and experience with chronic care models of health care
Experience with coordination of care of complex patients in various settings
