Join us and help people live healthier lives while doing your life's best work.(sm) Primary Responsibilities:Support Medicaid members in their homes and / or other settings, including community centers, shelters, hospitals or providers' offices Support Medicaid members with accessing mental health services, medical services, housing supports, and other connections to resources Assess and analyze the use of Behavioral Health case management services within the Optum SLCo Provider NetworkAct as Subject Matter Expert on case management services for those Optum SLCo Providers who are interested in developing and/or expanding case management services within their agencyAbility to communicate complex or technical information in a manner that others can understand, as well as ability to understand and interpret complex information from others
Required Qualifications:Bachelor's Degree or higher level of education in a healthcare (or related) discipline3+ years experience working with the Medicare/Medicaid patient population in a related healthcare environment 3+ years experience in Case Management Certification in Case Management or equivalent licensure qualification (not required for candidates with clinical licensures) Computer / typing proficiency to enter / retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other softwareReliable transportation and the ability to travel to required sites within the service delivery areaPreferred Qualifications:Master's Degree or higher level of educationExperience working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needs, Adult and / or Children with complex needsKnowledge/experience within the systems that provide services to vulnerable populations (DCFS, criminal justice system, inpatient facilities, homeless services, etc.)Bilingual Skills – fluency in English/SpanishProblem solving skills; the ability to systematically analyze problems, draw relevant conclusions and devise appropriate courses of action Careers with Optum.
