Position Summary SEIU 73
The Medical Social Worker III provides care coordination services for Cook County Health & Hospitals (CCHHS) patients who have been identified as at-risk for poor outcomes due to mental or physical disease, complicated treatment plans, functional inabilities to self-manage or poor social support. Care Coordination involves collaboration with patient and physician(s) to arrive at a treatment plan that is patient centered and consistent with best practices. Success is measured by patient engagement, stable or improved health status and improved health care utilization.
Licensed as a Social Worker (LSW) in the State of Illinois
Minimum one (1) year of clinical experience with a culturally diverse population
Minimum one (1) year of experience working with patients who have chronic medical or psychiatric illnesses
Minimum one (1) year of care coordination experience
Licensed as a Clinical Social Worker (LCSW) in the State of Illinois
Minimum three (3) years of ambulatory experience with culturally diverse populations
Fluent in Spanish or Polish
Knowledge, Skills, Abilities and Other Characteristics
Ability to use Microsoft Outlook
Ability to help patient set realistic and attainable goals
Knowledge of current standards in mental health, public health and public welfare, and Federal and State programs in these fields in order to provide advocacy and enhance patients' functioning/access to identified community resources in order to help them maintain acceptable levels of wellness
Demonstrate problem solving and conflict resolution skills
Skill to analyze situations accurately and take effective action in order to maintain safety and security of the Cook County Health and Hospitals System and the community at large
