Summary: This position is responsible for providing leadership, direction, and clinical support to Service Coordinators within Residential Services. This position also helps ensure delivery of person-centered services to all residential consumers. It requires 24-hour accessibility to provide direction to the service coordination team for residential. In addition, it will also require 24/7 on-call cell phone responsibility for medical, behavioral, and clinical emergencies regarding residential consumers. This position provides service coordination and 24-hour on-call availability for consumers in any Residential vacant Service Coordinator position; it requires a flexible schedule (i.e., days, evenings, and weekends).
Essential Functions * Provide leadership, direct supervision, training and clinical support to all Residential Service Coordinators while promoting a person-centered * Provide input into the development of departmental goals, guidelines, and practices to ensure efficient program operation. Track all necessary data for Consumer Services-Residential departmental goals and outcomes. * Ensure the delivery and/or coordination of all Consumer Services-Residential are in compliance with applicable licensing, accreditation funding bodies, agency mission, agency policy and procedure and best practice. * Assist with the development, implementation and maintenance of a quality assurance system for Consumer Services-Residential. * Collaborate with the Nursing department to ensure all appointments, including annual, physical, dental and follow up are being scheduled, completed, and the paperwork flow for Residential Service Coordinators is functioning well. Maintain contact with RN During the absence of a Residential Service Coordinator, serve as the Service Coordinator, ensuring policy compliance and quality care to consumers. Provide 24-hour on-call availability and emergency response for caseload. * Assist Director of Consumer Services and Director of Residential with the intake process, screening, program placement and appropriate movement of individuals served
Education * Bachelor's Degree in a human service field of study required.
Experience * Minimum of one year experience working with individuals with developmental disabilities and/or mental illness. * Prior experience in a residential setting, preferably in a supervisory role. * Supervisory experience preferred.
License/Certification * Certification by DHS as a qualified QIDP
Language * Second language a plus, but not required.
Knowledge of Rules and Regulations * Knowledge and understand of Rules 115 & 116 and assisting in the development and implementation of policies and procedures which assure compliance through service delivery. * Meet requirements of applicable licensing, funding, accreditation and other state/federal regulatory agencies; including safety requirements and agency policies and procedures, through utilization of systems to ensure compliance with all standards.
Additional Job Responsibilities * Assist in the hiring process for Residential Service Coordinators. * Provide leadership in the development of intervention plans for individuals in Residential Services. * Provide open and authentic feedback to staff on a regular basis related to performance utilizing active listening skills and allowing constructive feedback. Complete performance appraisals in a timely manner. * Meet requirements of applicable licensing, funding, accreditation and other state/federal regulatory agencies; including safety requirements and agency policies and procedures, through utilization of systems to ensure compliance with all standards. * Demonstrate the capacity to manage multiple demands; with flexibility in adapting to new challenges, while maintaining priorities. * Function as a member or facilitator of agency committees, as appropriate.
