SUMMARY OF JOB RESPONSIBILITIES:
This position is responsible for the infrastructure and systems that support patient care provided in the home setting. This is accomplished through:
Developing an annual work plan in collaboration with staff and other leaders.
Implementing the strategies outlined in the annual work plan – communicating with staff and leaders about progress and using data to measure success.
Serves as a resource on all issues of patient care and clinical support, demonstrating a working knowledge of policies and procedures, the philosophy of the organization, and state and federal rules and regulations pertaining to hospice.
Collaborating with other departments that complement the full services of hospice, Bereavement, Volunteers, Hospice Care Center, Medical Affairs, Admissions, HopeWest Kids, Patient Billing, etc.
Representing the needs of home-based care staff to other administrative departments such as Education, Information Services, Facility Management and Finance/Informatics.
Assuring quality of patient care through assessment of outcomes, including but not limited to CAHPS survey, HIS measures, clinical documentation, and
Using the online tools of the HealthCare Safety Zone Portal, reviews all reports incidents and concerns and assures prompt, thorough and timely follow up is done and documented.
Assuring an ongoing connection of clinical staff with the organization's mission and vision, including philanthropy.
Creating systems that provide for ongoing engagement, support and appreciation of clinical staff.
Serving as a liaison to other organizations in the community and connect to hospice, i.e. home health agencies providing private duty services to HopeWest patients.
Reviewing and confirming all hiring decisions to assure that hiring practices are aligned with the organizational goals.
Developing an annual QAPI plan for home-based hospice in collaboration with Quality Department which includes measures to monitor for compliance as well as quality.
Developing an annual budget for home-based hospice in collaboration with supervisor and Finance Department which includes staff productivity goals and direct patient care expense goals.
Routinely monitoring direct patient care expenses for appropriate utilization.
Developing and implementing goals for volunteer utilization on the teams supervised.
Assuring that the office environment is aligned with the needs of staff and the culture of the organization.
Participates in the Administrative On-Call rotation
Good computer skills, including use of internet/intranet and Microsoft office products, including Outlook, Word, Excel and PowerPoint.

RN with current Colorado license; will consider another clinical discipline with proven leadership experience
BS/BA required; Master's degree preferred
At least 3 years of leadership experience in a healthcare setting
Prior experience with effectively supervising others in a manager role
Excellent organizational, interpersonal and communication skills
Excellent problem-solving and decision-making skills
Valid Colorado driver's license and unlimited access to reliable transportation
Auto insurance that meets Colorado standards is maintained
