High School diploma/GED or equivalent work experience (3 yrs +)
Minimum Experience Requirements
Preferred: 6 months of CNA experience
Current Colorado Certified Nurse Aide (C.N.A.)
Colorado Driver's License
CPR = Cardiopulmonary Resuscitation (CPR) required, Basic Life Support (BLS) from American Heart Association required for fac
with an Automated External Defibrillator (AED) preferred for facilities without an AED.
Position Duties (Essential Functions)
Provides and/or assists with personal care needs of patients/residents as directed on Care Plan. Demonstrates clinical competence in the following areas: bathing, vital signs, hair, mouth, and nail care, caring for patients with tubes, foleys, I.V.'s, feeding tubes, assisting with ambulation, exercises, and
light housekeeping/meal preparation as assigned. Demonstrates competence in safe equipment use and maintenance. Demonstrates knowledge and compliance with policies. Demonstrates ability to perform duties in a variety of environments.
Communicates with nurse regarding patient's condition. Participates in patient care conferences. Communicates need for Supervisory Visit to primary nurse and assists with coordinating visit. Participates in quality processes through reporting of Customer Concerns and Occurrences and participation on work
teams. Notifies clinical leadership of any environmental concerns. Documentation Submits DAR's (complete) along with paperwork per agency timelines. Documents care provided (or reason why visit not made) in correct format. Visit frequency and duration is in compliance with physician orders. Supports operational needs related to documentation requirements both internally and externally.
Attends all mandatory/scheduled meetings. Maintains productivity per FTE status. Adherence to Centura Health Associate Principles/Human Resource policies regarding dress code, attendance, punctuality etc.
