Shift: Variable, 1, 8 hour shift.
The Client Advocate will work with other program staff to support the health and well-being of HealthSET's clients. Under
the direction of the Executive Director and Health Promotion Clinic Coordinator, the Client Advocate will provide direct,
non-nursing assistance to our clients.
HealthSET serves the independent living, low-income elderly. The seniors we assist live in rent-subsidized high-rise
buildings throughout metro Denver. A large percentage of the people we help live on only $600 or $700 per month. We
provide services to this population through Health Promotion Clinics, Advocacy, Community Health Coaching and Nurse
Our staff and trained volunteers give seniors wellness assessments; monitor health problems; provide health coaching;
educate residents about nutrition and other good health practices; and provide referrals to other area health care
HealthSET staff go directly to the community rooms of these high-rise buildings, so that seniors can readily access our
services. The ultimate goal of all of our programs is to empower these individuals to continue to live independently and to
feel that they are in control of their lives and health. HealthSET staff also provides advocacy to clients by: accompanying
clients to medical appointments; making follow-up calls, communicating with the client's primary care provider; and
referring clients to other community agencies to complete the system of care. These programs support older adults as they
strive to remain in their homes, cope with complex medical needs, psychosocial crises, and life transitions which require
direct collaboration with health care providers and diverse community resources.
Minimum Education Requirements
Bachelors Degree and/or equivalent and relevant experience
Minimum Experience Requirements
Knowledge of community based resources in and around Denver Metro area that are specific to low-income seniors preferred
Flexible within an ever-changing environment
Capable of optimizing time and efficiently working with clients in a face-to-face setting
Must be comfortable measuring and reporting on client progress
Strong computer skills with experience in Microsoft Word and Excel
Background in social services, and/or health care familiarity
Community advocacy and ability to network with other community based organizations
Ability to recognize general changes in emotional or physical well-being of clients
Desire to serve the poor and elderly
Patience and compassion

Must possess valid Colorado driver's license and appropriate auto insurance as required by law
Position Duties ( * denotes an essential functions of the position)
Working directly with clients to advocate for their health and needs working with community agencies to connect clients to available resources.*
These may include: assisted living organizations, hospice, legal services, benefits eligibility, mental health referrals for individual or group counseling, home and community based services, visiting with and attending appointments, arranging services such as meal delivery, transportation, in-home care, family and caregiver coordination and communication around best care for client and follow through on care regarding medical procedures.*
Provide support for the Adopt an Elder program. *
Working collaboratively with Resident Service Coordinators located in the 20 lowincome high-rise apartment buildings HealthSET serves. *
Assist with Health Promotion Clinics as intake worker or support for community resources.*
Implement health education programs in HealthSET supported buildings.*
Complete current certification for certifying documents and enrolling clients. *
Attend all HealthSET and Centura trainings/webinars as required.*
Keep accurate statistical data and reporting systems up to date.*
Monitor effectiveness of services for clients and advocate for or assist in the development of new services as needed. *
Provide monthly, quarterly, and annual reporting.*
Reporting will include information about outreach activities, number of people served, amount of time spent, and type of services provided.*
