Florida Atlantic University's College of Nursing is seeking a Nurse Practitioner, Boca Raton, FL.
The Gerontological Nurse Practitioner will initiate and lead the comprehensive memory & wellness evaluation clinical process conducted by an interprofessional team of geriatric specialists in consultation with the patient's personal physician/health care provider.
The evaluation will address the nature, extent and cause of memory problems.
Perform an extensive, memory-focused history and physical assessment. The visit also includes a review of all current medications, a screening of emotional factors that may affect memory, and a screening of cognitive functions, including memory.
Family members/caregivers also have the opportunity to meet with the GNP and with one of our team members to raise their concerns and questions.
Remain attentive and responsive to the patient's and the family member's/significant other's needs.
Work with the patient's existing physicians to obtain necessary laboratory work and neuro-imaging studies.
Determine next steps into the process, such as appropriate referrals for neuropsychological testing and supportive services.
Meet with the patient and family at the feedback session to review the test results and recommendations. Evaluation reports are then forwarded to the patient's treating physician(s)/health care providers.
Our visits take place at the FAU Memory and Wellness Center and also in the community (homes, ALFs, etc).
Master's degree in nursing and one year of appropriate experience required.
Science Board Certification preferred.
AGNP/GNP BC preferred.
Licensure as an advanced registered nurse practitioner (ARNP) in accordance with Chapter 464, Florida Statutes- Required.
Bilingual: English and Spanish-Required.
Valid driver's license required.
