We are seeking an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) for the Ambulatory Benign Hematology and Anti-Coagulation team. Responsibilities include working collaboratively with the designated members of the interdisciplinary team and using advanced nursing skills to manage the care and treatment of a caseload of patients with benign hematological disorders and patients requiring anti-coagulation management in the outpatient setting. He/She will participate in shared and sole provider visits, the implementation of patient education, and clinical support programs to assure optimum care throughout the healthcare continuum.

Qualifications for NP are
Current NYS RN and NP license/registration
Prescriptive privileges (or eligible)
A minimum of 3-5 years of clinical experience
AACN certification required
Hematologic experience strongly preferred
Qualifications needed for a PA candidate are:
Graduate from an accredited PA program
Active NYS license or be qualified for state licensure
A minimum of 2 years of related experience as a PA is preferred
Hours: 10 hour days flexed Monday – Friday
