Coordinates case management services to improve thepatient's ability to function in the community. Services are performed toimprove the skills and abilities of patients to live independently in thecommunity, improve self-care and social skills, and promote lifestyle changeand recovery practices. Provide rehabilitation services individually or ingroup sessions. These services are goal oriented outpatient interventions thattarget the maximum reduction of mental and/or behavioral health impairments andstrive to restore the patient to the best possible mental and/or behavioralhealth functioning. Works collaboratively with multi-disciplinary team to plan,evaluate and plan individualized treatment goals. Assist with admission andcontinued stay reviews concurrently and retrospectively.
Provide Behavioral Health Case Management and Behavioral Health Rehabilitation services. Develop and perform rehabilitation group materials which are specific to the adolescent population. Documents and participates in the program by collecting data for assessment and reassessment in patient record. Documentation is clear, legible and accurately describes any changes in patient condition, treatment progress/goals/interventions, therapeutic/milieu groups or patient progress notes, and other patient safety documentation duties.
Demonstrate knowledge of the principles of growth and development and proficiency in assessing, treating, and caring for patients according to their age-specific needs.
Apply knowledge of group dynamics in participating in therapeutic groups and maintaining the milieu.
Initiate delegated tasks based on demonstrated competence in performing program activities for patient population served. Apprise Licensed Mental Health Therapist (LMHP)/ Registered Nurse of any changes in patient condition, of patient complaints and of patient/significant other educational needs.
Orient new patients and significant others to the program and prepare patients for discharge.
Facilitate and/or participates in assigned or scheduled milieu functions including rehab, therapeutic groups, outings, etc. unless reassigned other duties by charge nurse. Assist Therapists and Teachers in groups/school/activities by actively promoting, encouraging, and motivating patient's participation.
Assess data to determine medical necessity and appropriateness of level of care/admission based on approved criteria. Complete insurance verification and authorizations for specific pt population from preadmission through discharge.
Attend treatment team meetings with physicians, therapists, and nursing staff to discuss treatment options, benefits, insurance authorizations, and discharge planning.
Collect, record, and maintain clinical information gathered from concurrent or retrospective review and assess for medical necessity criteria. Utilize EPIC for documentation purposes.
Display self-motivation through designing, planning, and initiating work processes and activities with minimal supervision including Case Management and Rehabilitation Services. Attends and actively participates in departmental, hospital and medical staff meetings as assigned.
Perform other related duties as directed that correspond to the overall function of this position.
Bachelor's or Master's degree in a behavioralhealth related field from an accredited college or university recognized by theUnited States Department of Education (USDE) or an Educational degree earnedfrom an accredited college or university recognized by the (USDE) with at leastnine (9) hours of college credit in a behavioral health related field required.
Certification from Oklahoma Department of MentalHealth and Substance Abuse Services ODMHSAS as a Case Manager II required. Ifincumbent is a Licensed Mental Health Professional under supervision for theirlicense or a Certified Alcohol and Drug Abuse Counselor (CADC), ODMHSAScertification is not required.
Current CPR (Heartsaver).
Must be 21 years of age. Any unit specifictraining must be completed in accordance with the unit policies and procedures.
Previous behavioral health experience withadolescent population preferred.
