Must have Case Management Certification - CCM, CIRS, CDMS or CRRN

Summary: Provides disability case management, clinical and group work services by performing the duties listed below.
Manage all disability cases including Short Term Disability and referrals to Long-Term Disability.
Counsels employees assisting them to identify and resolve issues relating to their specific case as well as company policy and procedures.
Acts as liaison between employee and providers if needed
Assess and evaluate cases and prepare and implement case management plans.
Assesses the coordination and access to needed medical, psychiatric, social, educational, and other services essential for meeting the health needs of the employee
Assists employees with finding ways of adjusting to changes with physical capabilities that will allow for a life/work style as comfortable as the situation will permit.
Provide consultation and participate in meetings with employees, managers, Human Resources, Legal, and Employee Relations regarding sensitive employee issues when needed
Prepares and maintains written case records, reports, and forms, performs case follow-up until closing, and performs other administrative tasks as required
Provide written notifications to employees and managers regarding the status of each case including approvals, changes and FMLA notifications.
Coordinates return to work processes with the employee, providers, and employer
Manage compliance with corporate policies and procedures, the Family Medical Leave Act and the American with Disability Act.
Manage Workers Compensations cases and work closely with insurance carrier to resolve claim issues and coordinate time off with other company leave plans.
Manage Job Modification process and assist in the process to make the determination of covered conditions under the Americans with Disabilities Act.
Assist with coordinating on-site workstation evaluations.
Manage corporate lactation program.
Completes mandatory OSHA reporting
Enter approved leave time into time reporting system requiring knowledge of company's policies and procedures and FMLA compliance.

While working at this employer site, you will have the ability to get to know your patients, while helping to improve their health status and taking care of their immediate health needs.
Associates'degree (A.D.) or equivalent from a two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination or education and experience.
Field case managers must maintain a valid state driver's license
Current certification in ARC or AHA Basic Life Support required
Certified Insurance Rehabilitation Specialist (CIRS), Certified Disability Management Specialist (CDMS), or Registered Nurse with Certification in Rehab (CRRN) or Case Management (CCM)
Knowledge of Internet software, Spreadsheet software and Word Processing software
Bachelor's degree in Nursing (BSN) from four-year College or University is preferred.
Knowledge and experience with Electronic Medical Records.
Knowledge of FMLA, ADA and Workers Compensation regulations and compliance.
