Under close supervision, the clinical social worker performs full range of social work duties and responsibilities for the UCSD Mother, Child & Adolescent HIV Program. Primary responsibilities include conducting comprehensive psycho-social assessments and formulating care plans, providing crisis intervention, intensive case management and coordination of treatment services for HIV positive patients and relatives. Participate in patient care conferences with team members and coordinate implementation of interdisciplinary plans. Through individual, couple, or group counseling interventions, assist infected and affected individuals in adjusting to the physical/psychological challenges associated with HIV. Assist patients' access health care benefit programs and community resources. Ensure that all treatment plans and delivery comply and adhere to federal, state, and local social work code; delivering care ethically and in non-judgmental and non-discriminatory manner, with sensitivity to the patient's cultural, religious, and/or age specific needs; and preserving/protecting patient autonomy, dignity, and rights and follows the Patient Bill of Rights. Professional and community activities include participating in program and community committees; developing and/or coordinating special projects and/or programs for patients, seeking grant funding and participating in grant writing. Administrative responsibilities include participating in quality assurance and program evaluation activities, and maintaining departmental statistics. Other professional activities include participating in community planning, developing and/or coordinating special activities for patients, maintaining clinical files, and generating departmental reports.
A Master's degree from an accredited school of social work; or an equivalent combination of education and experience; and knowledge and abilities essential to the successful performance of the duties assigned to the position.
Demonstrated skill in social work assessment of and interventions with individuals and families impacted by chronic disease, loss, and threat of death. Strong crisis intervention and counseling skills.
Experience working with children, adolescents and adults in a medical or social service setting.
Demonstrated experience with social work interventions and required documentation in cases of suspected child abuse/neglect, domestic violence, elder/dependent adult abuse, chemical dependency and mental health crises.
Demonstrated experience integrating theory and practice in the formulation of psychosocial assessment and interdisciplinary treatment plans, the implementation of care plans and the modification of treatment based on outcomes.
Demonstrated experience conceptualizing and writing clear, concise, appropriate psychosocial assessments, care plans, chart notes, and progress reports in social work and medical records.
Demonstrated ability to deliver care in an ethical, nonjudgmental, culturally competent, and nondiscriminatory manner.
Ability to converse fluently, read and write in Spanish.
Knowledge of the physical, emotional, social, financial impact of HIV/AIDS on infected and affected individuals, couples, and families.
Knowledge of electronic medical record; Epic, Appointments Pro scheduling software, AIDS Drug assistance program ADAP eligibility and certification processes.
Knowledge of and demonstrated adherence to federal, state, local laws related to health and social welfare including, but not limited to, Medi-Cal, Medicare, California Children's Services.
Demonstrated experience preparing and professionally delivering educational presentations to consumers, professionals, and lay audiences.
Must be willing and able to travel to client homes, community meetings, and occasional out-of-town conferences.
Must have a valid California Driver's license.
Must have access to reliable personal transportation with proof of vehicle insurance.
