Some weekends will be required with rotating on-call.** Primary Responsibilities:Plans and administers medically prescribed occupational therapy treatment for patients suffering from injuries, or muscle, nerve, joint and bone diseases, to restore function and prevent disability Reviews referral, physician's orders, patient's' condition and medical records to determine occupational therapy treatment indicated. Accepts physician orders regarding occupational therapy services Assists the physician in evaluating the patient's level of function Performs initial patient evaluation before initiating therapy. Performs and documents OASIS assessments at appropriate time points Assess the patient's home for safety. Instructs the patient on home safety as indicated Plans and prepares written treatment plan to regain upper mobility function, fine motor skills and assist with returning patient to optimal independence with ADLs based on evaluation of patient data in conjunction with the physician, patient and caregiver(s) Prepares a written plan for the occupational therapy assistant and / or Home Health Aide to follow, if applicable Coordinates participation in sensorimotor, educational, recreational and social activities designed to help patients develop or regain physical functioning to adjust to disabilities Selects constructive activities suited to the patient's physical capacity, intelligence level and interest to maximize independence, prepare patient for return to employment, assist in restoration of function and aid in adjustment to disability Instructs, motivates, and assists the patient to learn and improve functional activities and daily living activities and in the use of assistive and supportive devices Designs and constructs special equipment for the patient and suggests adaptations of the patient's living environment to promote independence. Makes referral to obtain necessary ready-made equipment Identifies and addresses barriers to desired outcomes Sets goals of therapy in conjunction with patient / caregiver(s) and physician and evaluates the effectiveness of treatment and outcomes of care, adjusting treatment as needed to achieve maximum benefit Performs care management duties on patient whose primary skilled service is occupational therapy including: notifying the physician of changes in the patient's condition or progress toward goals, obtaining needed occupational therapy orders, reassessing the patient for recertification, supervision of the home health aide, attending and documenting case conferences, coordination of care by reporting of significant findings to others on the healthcare team, and planning for, notification of and documenting the discharge of the patient Confers with physician and other practitioners to obtain additional patient information and suggest revision in treatment program and to integrate occupational therapy with other aspects of the patient's health care Orients, instructs and supervises work activities of assistants, aides and students Advises and consults with patients, caregivers and other staff members Attends case conference and staff meetings Coordinates services and scheduling with the Clinical Team Manager (CTM) and Clinical Team Coordinator (CTC) to include recommendations for additional home health care services for patients within 24 hours of Start of Care (SOC) Documents patient interventions that reflect delivery of safe and high-quality home health care on a visit note written during the patient encounter. Turns in all documentation within 48 hours of the visit (visits performed on Thursday, Friday or Saturday are due on Monday) Documents all communications with the patient, family, physician, CTM, pharmacy, other disciplines, and appropriate others as indicated on communication notes. Participates in in-services and educational programs Manages and supervises COTA employees at least monthly. Supervises all care rendered by the COTA and co-signs COTA notes. Responsible for the overall direction, coordination and evaluation of their work Demonstrates knowledge and observance of the Patient's Bill of Rights Follows all infection control, standard precautions, and safety guidelines / standards per agency policy Complies with all Homecare Dimensions, Inc. policy and procedures Promotes and maintains an agency environment that is in compliance with federal, state, and local regulatory agencies Participates in the agency's endeavors for accreditation, licensing and professional recognition according to state, federal or CHAPs requirements Participates in home care Performance Improvement Program activities. Participates in periodic review of clinical records Demonstrates personal responsibility with regard to attendance and punctuality Maintains privacy and confidentiality with regard to all patient, staff and agency information Promotes the agency image by adhering to the Dress Code Meets productivity requirements based on agency standard Follows the appropriate chain of command in all forms of communication Demonstrates flexibility, enthusiasm and willingness to cooperate while working with others or in place of others as necessary
Required Qualifications:Graduate of an accredited school of occupational therapy approved by the Council of Medical Education of the AMA in collaboration with the American Occupational Therapy Association. Registered by the American Occupational Therapy Association 2+ years institutional experience Currently licensed as an occupational therapist by the Texas Board of Occupational Therapy Examiners Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals Ability to write routine reports and correspondence Ability to speak effectively before groups of customers or employees of Homecare Dimensions, Inc. Ability to define problems, collect data, establish facts, and draw valid conclusions Ability to interpret an extensive variety of technical, medical, and regulatory instruction and deal with numerous contemporaneous issues and problem solving, delivery of patient care, and management of a diverse and multidiscipline staff applying abstract and concrete variables Maintains certification in Basic Cardiac Life Support Course C Familiarity with medical terminology and equipment Extensive periodic travel within the agency's service area Ability to transgress entries and exits, to transport self from place to place using an automobile and accomplish personal locomotion from place to place Must show proof of valid automobile liability insurance At least 90% of time away from office Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area This position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor's diagnosis of diseasePreferred Qualifications:Ability to work in a stressful environment Positive work environment is promoted for all agency employeesCareers with WellMed.
