Harborview Mental Health and Addictions Services has an outstanding opportunity for a full-time Program Coordinator to assist in the coordination of care for medication-assisted patients recovering from opioid addiction. The Coordinator works in cooperation with other programs including Harborview Addictions Program clinic and Harborview Mental Health Services, Pioneer Square Clinic, and others in support of patient recovery. This position coordinates the delivery of medical care, mental health, recovery and other social services for patients requiring medication-assisted treatment for opioid use disorder and access to health care. The Coordinator facilitates and tracks the patients' entry into and through the spectrum of clinical services of the health care system, as well as makes referrals to a spectrum of other services. This position is also responsible for coordinating outreach [AB2] and engagement with the peer navigators, the nurse care manager and other multidisciplinary team members, to provide patient care.
Primary Job Responsibilities:
Conducts outreach through email to HMC clinics, providers to offer linkage services to patients identified as targeted client base.
Coordinates with personnel from Public Health Seattle & King County, other treatment programs and area clinic managers and medical providers to provide information on MAT and related services and to coordinate with regard to clients identified as needing assistance with MAT.
Coordinates with case managers and inpatient medical providers to assess need for linkage to MAT care among persons hospitalized in HMC or in aftercare.
Collaborates with providers and staff of out-patient clinics to inform them about MAT and coordination of various health, social service and recovery activities
Handles intake of all referral requests, ensuring complete information is received and entered into the computer system. Provides telephone screening of patient medical problems and severity of condition. Triages patient telephone calls to clinical staff; initiate emergency procedure when necessary. Coordinates incoming and outgoing referrals,
The Coordinator provides information and advice regarding treatment services to prospective patients and their advocates, to personnel from outside agencies, staff from outside hospitals, and the general public in addition to the patient population.
Acts as primary contact for patients being referred to MAT services. Provides messaging and instructions to patients about MAT, about screening, and treatment. The Coordinator implements a screening questionnaire to each prospective patient to assess eligibility and fit for the program. Interviews patients and reviews anticipated needs upon admission to the program and discharge, including: personal escort, transportation, housing, support care, equipment needs, transfer of records, and appointments with physicians and care managers.
Ensures complete and accurate registration of patients to receive MAT services, including patient demographic and insurance referral information are in the computer system. Ensures a complete medical record is present at the time of patient visit, admission, and/or procedure. Manages Epic referral queues and other databases.
Educates clients as needed about related recovery programs and may inform patient about mental health services, social services and other programs or services related to their recovery. Coordinates referrals and follow-up activities as needed to link clients with recovery-related support, mental health, medical and financial and health care coverage services. Appropriately directs patients and others to other departments across the medical center. Schedules various appointments for patients.
Under general direction and using knowledge of services, independently advises patients and prospective patients on accessing health care. Has extensive involvement with new and returning patients, case managers, nursing staff, physicians and other clinic staff in carrying out program activities.
Coordinates clerical patient care activities and/or specialized technical duties unique to the assigned clinic to assure accurate and expedited patient check-in, including obtaining consents for care and referral information. Assists with completion of patient forms, applications, and other paperwork. Completes appropriate documentation for referral to clinic programs. Facilitates the flow of documents between patient, providers and payers. Facilitates sending of treatment summaries to patient's primary care physician.
Oversees health care efforts through assistance managing patient registries and provider panels and patient outreach processes in coordination with attendant HMC clinics and outside resources.
Reviews appointment follow-up requirements and expectations with patient to ensure adequate and timely review of the patient plan of care in accordance with all CMS and other regulatory requirements.
Acts as resource to the team for payor issues including insurance knowledge for Medicare, Medicaid and commercial plans. Facilitates prior authorization with insurers for payment of treatment. Coordinates with the case managers, financial counseling and the contact center for verification of benefits and authorization for outpatient visits. Refers patients to Financial Counselors when needed.
Works with clinic care teams in supporting clinical activities related to recovery, mental health, chronic care and other health care services (running reports, identifying care gaps, calling patients, making appointments, obtaining screening results, entering data into the medical record.

High school graduation or equivalent AND two years of experience in the program specialty OR equivalent education/experience.
Desired: A minimum of two years of experience as a Patient Services Specialist 2.
