Advertised Summary Job Description: Reporting to the Director of Care Management, Incumbent will provide patient navigator services and professional administrative support to the Care Management program at CUMC. Work closely with the Care Management team to assist in their workflows. Manage the daily activities of the Care Management group and serve as office manager, including but not limited to: coordinating patient care, helping with care gap closures, coordinating meeting schedules; managing high volume phone lines, responding to routine inquiries and providing information follow-up support.
General Minimum Qualifications: Requires a bachelor's degree or equivalent in education and experience, plus four years of related experience.
Additional Specific Minimum Qualifications: Computer proficiency in MS Office (Word, Excel, PowerPoint, Access) Excellent written and oral communication skills. Strong interpersonal and organizational skills a must. Friendly, professional and helpful phone mannerisms required. Must be able to work under pressure both independently and in a team setting. Must have a pleasant, professional demeanor and the ability to interact effectively with a wide variety of people. Must be able to function in a highly fast-paced environment. Ability to establish priorities and follow up accordingly. Must maintain confidentiality. Spanish speaker.
