University Hospital is the Health System's hospital for adult patients. The 11-story, 550-bed hospital first opened its doors in 1986. Today, 70 percent of University Hospital's patients are admitted from communities or regional hospitals outside the Ann Arbor area. Care Management Social Workers are assigned to each unit within the University Hospital and are responsible for all inpatient social work duties on their assigned units.
The Department of Care Management is seeking a part-time (0.5 FTE, 20-hour/week), licensed Social Worker for an inpatient unit in University Hospital. The Social Worker will assess clinical and psychosocial needs, as well as coordinate transition planning for the unit, including discharge planning as needed.
Care Management Social Work services include but are not limited to the following: crisis intervention, family support and counseling, end-of-life counseling and support, hospice initiation, grief and loss, accessing resources to facilitate hospital transitions and/or discharges, linkage to community agencies, mental health assessments, tangible resources, participation in team meetings and educational activities related to needs of patients and their families.
A Master's degree (MSW) in social work from a school accredited by the Council of Social Work Education is required. Candidates must have a current clinical license (LMSW or LLMSW) to practice Social Work in the State of Michigan. It is essential that the candidate be able to manage a large caseload with multiple competing demands. The ability to work independently in a variety of stressful and demanding situations is essential; critical decision making, excellent assessments, brief interventions and multi-tasking are only several of the advanced skills required for success. Additionally, the ability to provide high-level interventions while also meeting basic tangible and concrete needs is required. The successful candidate must also be able to work with multidisciplinary teams and access community resources in order to facilitate discharge planning. Experience providing crisis intervention, grief counseling, and end-of-life discussions are essential, along with brief interventions to facilitate patient and family participation in appropriate health care decision making.
LMSW is strongly preferred. The ideal candidate will have significant medical social work experience (preferably inpatient, hospital-based), strong assessment skills, the ability to establish therapeutic relationships quickly, and to relate well to a diverse population. Clinical practice concentration and field practicum or recent work experience in a hospital preferred.
Hours are 8:00 AM - 5:00 PM, rotating schedule: 2 days one week, 3 days the next week. Holiday rotation coverage is required.
