Vanguard Medical Group has an exciting new opportunity for a full time or part time Psychiatric Nurse Practitioner splitting time between our North Jersey offices. The Psychiatric Nurse Practitioner will work as a part of the multidisciplinary health care team in the primary care setting, focusing on the behavioral health needs of the patient. The Psychiatric Nurse Practitioner supports the patient-centered medical home by working with patient, families, staff providers, non-staff providers and ancillary staff to promote timely access to needed care, providing daily continuity of care. The Psychiatric Nurse Practitioner will provide culturally competent, age and education level appropriate behavioral health care. Patients, families and caregiver’s, when appropriate, understanding will be assessed when providing a plan of care and self-management responsibilities.
Collaborate with the medical providers to develop programs and working relationships that promote shared approaches to the evaluation, treatment planning, and monitoring of common mental health conditions
Provide a variety of therapeutic mental health treatments including, but not limited to counseling, Cognitive Behavioral Therapy and initiation, titration, and monitoring of psychiatric medications
Establish and modify patient treatment plan based on clinical assessment as well as accepted standards of practice, cultural competence and patient’s understanding.
Document responses to those treatments
Formulate differential diagnoses
Provide episodic visits to monitor acute and chronic mental health conditions.
Master’s Degree in Nursing from an accredited school
Certification by the American Academy of Nurse Practitioners and/or American Nurses Credentialing Center is required
Licensed or certified Advanced Practice Nurse/Nurse; practitioner (APN) to practice as a Psychiatric APN
Currently licensed by the New Jersey Board of Nursing as a Registered Nurse
Must have prescriptive privileges in the State of New Jersey
Must be detailed-oriented, possess excellent communication, interpersonal and time management skills
Familiarity with Electronic Health Record (EHR) required
At least two years of experience in behavioral health field either acute or primary care
Ability to apply critical thinking skill and make sound judgments both while performing daily responsibilities and throughout the patient’s continuum of care
Knowledge of the patient centered medical home (PCMH)
Knowledge and demonstrated abilities to work in a regulatory climate that includes oversight by federal and state rules, payer contracts, governmental benefits and community resources
Effective oral and written communication skills
Excellent interpersonal skills reflecting clarity and diplomacy and the ability to communicate accurately and effectively with all levels of staff and management
Detail-oriented, thorough and able to handle multiple talks and projects with varying deadlines and priorities
Ability to work successfully in a fast-paced environment
Ability to work with an electronic health record
Possess empathy, mental alertness, precision and analytical problem-solving abilities, communication skill focus and initiative
