The Case Management Assistant (Principal Access Rep) coordinates referrals necessary for the patient's discharge plan as developed and identified by the Clinical Case Manager. Upon the request of the Clinical Case Manager, the Case Management Assistant completes the referral process to agencies that will provide essential services to meet the post-discharge needs of the patient. These may include, but are not limited to the following: necessary durable medical equipment (DME), home care services, home health nurse/aide, physical therapy/occupational/speech therapy, hospice and coordination of patient/family home infusion, enterals, etc. This position promotes the integration of a timely discharge plan through effective communication with the entire interdisciplinary team, patient, family, caregivers and external vendors and providers. This position is responsible for appropriate documentation in the patient's medical record of the efforts made in coordinating discharge services.REQUIRED - Knowledge of health insurance regulations, policies and procedures, Medi-Cal, MediCare Guidelines, private insurance and managed care operations.- Typing skills and working knowledge of computers, printers, copiers, and fax machines.- Knowledge of computers and online applications, including Epic and Allscripts applications, web-based search engines, and other software programs, including Microsoft Office, (i.e. Powerpoint, Word, Excel and Access).- Ability to read and comprehend English text and medical terminology; knowledge of medical and nursing treatment plans, levels of care.- Ability to organize and direct varied health care team professionals in a manner that establishes a good rapport and professional working relationship.- Good organizational skills, ability to independently coordinate a variety of tasks and functions, ability to set priorities, and recognize situations requiring clinical intervention, ability to be flexible and responsive.- Knowledge of medical resources in the community and ability to coordinate those resources to assist with appropriate placement of patients discharged from UCDMC.- Competent decision-making and problem solving abilities and adept at initiating actions and effective resolution and follow through to ensure appropriate utilization of clinical resources for patient care, completion of unit activities and/or projects, demonstrating good judgment and common sense; analytical skills to review all available information, draw conclusions, develop and evaluate alternatives, develop solutions, and monitor processes.- Physical and emotional stamina to regularly work within tight time frames and critical deadlines; to deal with upset, irate or hostile members of the public; to handle sensitive and confidential information; to perform tasks requiring long periods of concentration.PREFERRED - Experience as LVN, MS, CAN, EMT and/or acute hospital experience with a strong understanding of medical terminology. - Knowledge and understanding of medical practices and procedures, medical terminology, disease categories and services provided in an ambulatory and institutional setting.SPECIAL REQUIREMENTS - THIS POSITION MAY BE SUBJECT TO A CRIMINAL BACKGROUND INVESTIGATION, DRUG SCREEN, LIVE SCAN FINGERPRINTING, MEDICAL EVALUATION CLEARANCE, AND FUNCTIONAL CAPACITY ASSESSMENT- Must be able to work occasional overtime and/or flexible hours as needed by the department.
