The Outpatient Sickle Cell Clinical Social Worker provides assessments, counseling, education, and advocacy services to primarily pediatric, adolescent/young adult, and adult patients, their families and caregivers. This position provides support to patients in clinic and infusion with hemoglobinopathies, including sickle cell disease and beta thalassemia. Patient populations range in age from infant to young adult, with a focus in assisting the pediatric population to successfully transition into young adulthood and the management of their chronic disease. In addition, the Sickle Cell Social Worker provides education and support services to medical staff and the medical community regarding the psychosocial impact of Sickle Cell Disease. This position will also provide consultative support to hematologists, and will respond to emergent psychosocial needs of both sickle cell and hematology oncology patients. The incumbent provides back up for pediatric oncology LCSWs and will need to meet eligibility for CCS paneling.REQUIRED- A Master's Degree from an accredited School of Social Work.- Clinical work experience in a social service or healthcare setting (i.e. case management or social worker).- Unlicensed candidates must be registered with the Board of Behavioral Sciences and working toward LCSW licensure.- Minimum of two years post-graduate social work experience.- Experience in taking psychosocial history and case management evaluations, including interview techniques, suicide assessments, and forensic designations.- Knowledge and skills in the areas of individual, group, and family therapy.- Basic understanding of community resources.- Demonstrated awareness of, and sensitivity to, dealings with cultural and socioeconomic diversity.- Basic understanding of computer and MS functionality.- Proven ability to manage conflict.PREFERRED- Social Work experience in an outpatient oncology setting highly preferred.- Possession of a CA Clinical Social Worker License (LCSW), active and unrestricted.- Demonstrate basic knowledge and understanding of hematological disease (sickle cell) and its impact upon patients throughout the lifespan.SPECIAL REQUIREMENT - THIS POSITION MAY BE SUBJECT TO A CRIMINAL BACKGROUND INVESTIGATION, DRUG SCREEN, LIVE SCAN FINGERPRINTING, MEDICAL EVALUATION CLEARANCE, AND FUNCTIONAL CAPACITY ASSESSMENT- Must be able to occasionally flex time to handle emergencies and work weekends or other shifts as assigned. - Must be able to travel between building on hospital campus and off-site as needed.
