The YWCA of Metropolitan Chicago is seeking CASE MANAGERS to enhance consumers’ experience, wellness and independence through assessment, advocacy, planning, communication, education, resource management and service facilitation.
These positions are based at our Chicagoland locations that include Addison, Chicago Heights, Jackson Blvd, and Cottage Grove in Chicago, and report to the Sexual Violence Support Services Clinical Program Supervisors. This is an exempt, full-time position; salary is commensurate with experience. Provide direct services, using a variety of approaches, that provide consumers with a valued experience and supports their journey towards wellness and self-fulfillment
Assess consumers’ basic needs for emergency resources; physical and emotional health; and other wellness components, such as social, occupational, and intellectual
Collaborate with consumers to develop and implement goal-oriented and integrated plans, based on consumers’ overall needs and interests
Assist consumers in acquiring basic elements of self-sufficiency: ID’s, medical coverage, birth certificates, U.S. social security cards, Employment Authorization Documents, etc.
Identify and assist consumers with accessing private and public resources and provide enrollment assistance, as needed
Work collaboratively with therapists, advocates, community organizations, parents/guardians and others to carry out service plans, maintain consumer stabilization and advocate for consumers’ overall needs
Maintain up-to-date records with the entry of consumer and service delivery information into database systems
Create, cultivate, and deepen strategic partnerships with public and private organizations and groups to establish valuable and responsive referral connections
Build internal partnerships, working collaboratively across the enterprise to promote consumer awareness of internal resources and create a seamless service delivery system.
BA, BS in Psychology, Social Work or related Human Services degree
Bilingual English/Spanish skills are required for three of the four available positions
Ability to complete a minimum of 40 hours in Sexual Assault Crisis Intervention Training within first 30 days of hire
Demonstrated knowledge of various entitlement programs and community resources
Entrepreneurial thinking and creative problem-solving skills
Strong verbal and written communication skills to include keen listening ability and strong relationship building skills
Self-directed, initiative driven, requiring little oversight
Proven track record of effective follow-up and follow-through
