Position Summary: We have an exciting opportunity to join our team as a Nurse Practitioner.
In this role, the successful candidate Coordinates and performs diagnostic, therapeutic and preventive patient care activities. Provides for and assures comprehensive nursing care through use of nursing process and appropriate standards of care. Functions within a collaborative practice agreement established with Division Chief. Experience or strong interest in research and assisting with clinical trial enrollment is essential.
Help to promote and support programmatic development in the areas of COPD and asthma.
Perform initial patient screening including comprehensive history and physical examination and work collaboratively with the physicians to implement a treatment plan.
Must be able to see patients independently as well as in collaboration with physicians.
Assist with patient phone calls; appropriately address patient concerns and questions, reliably identifying urgent patient matters and necessity for further evaluation.
Function as an asthma and COPD educator including, but not limited to the following:
Assessment of intrinsic and extrinsic factors which may be triggering exacerbations of these conditions.
Phenotyping of asthmatics to assess both triggers and potential role for biologic therapies.
Assessment for co morbidities including but not limited to GERD, obesity, heart failure and sleep disordered breathing.
Administration of ACQ (asthma control questionnaires) and CAT (COPD assessment tests).
Review of proper use of inhalers at each visit.
Implementation of asthma action plan
Order appropriate tests and laboratory studies, analyze the results and determine the need for appropriate follow up appointment and further treatment options.
Prescribe appropriate pharmacologic and non-pharmacologic treatment modalities.
Follow up of all laboratory tests with patients in a timely manner.
Administration of all biologic therapies.
Assist staff with clinical support needed to ensure appropriate and timely authorization for biologic therapies.
Minimum Qualifications: To qualify you must have a Master's Degree in Nursing from an accredited program plus five years clinical nursing required. Current NY R.N. license and New York State nurse practitioner certification with prescription privileges is also required. Accreditation from National Certification Body, (AANCP) Completion of an approved pharmacology course, a written collaborative practice agreement and practice protocols developed with supervising physician required.
