The Nurse Practitioner will be responsible for:
Assisting in building our growing International Center with our group of dedicated Case Coordinators who work with patient's primary care physicians, corresponding department, specialty clinic RNs and/or medical staff to: develop in-take inquires, triage emails/calls, coordinate patient access, and help inform and update clinical communications to Embassies.
Building programmatic clinical infrastructure in conjunction with International Operations and Nursing leadership.
Providing oversight and coordination of care of all international patients managed within their defined international center team.
Facilitating Intake process by providing more comprehensive clinical oversight.
Helping to identify/anticipating all outpatient appointments required for pended inbound patients.
Reviewing clinical itinerary for all complex cases (complex= 3 or more program/department/division consults)
Participating on IMS rounds.
Assisting in navigating any questionable referrals to the appropriate BCH department, clinic or specialty program. The nurse will be alerted to urgent concerns of the families and/or the referring clinicians require nursing review; and will assess, analyze and plan appropriate interventions by working through their international team, to ensure that all parties are made aware of the planned interventions.
To qualify, you must have:
A Master's in Nursing (FNP/PNP)
Case management experience and assisting in the care coordination of patients with complex medical needs requiring coordination of multiple appointments.
5+ years of pediatric RN experience and 2-3 years of nursing at Boston Children's Hospital required.
NP experience strongly preferred, but will consider new NP's with the above experience.
Experience managing pediatric complex care, and/or outpatient clinic experience preferred. Strong leadership and communication skills required. Program management and supervisory skills preferred.
Must be highly organized with strong written and verbal skills.
Fluency in a second language preferred.
