The Clinical Pharmacist is responsible for both direct and indirect patient care, drug therapy monitoring, formulary management, staff education, patient consultant, cost containment and serves as Pharmacy's liaison with physicians and other health practitioners. Interfaces with physicians and other members of the health care team in the design of treatment regimens, and serves as an authoritative information source on drugs and their utilization in therapy. The chosen candidate will work in collaboration with the Clinical Pharmacy Team, Disease Management, Case Management, and High Risk Touch Team to ensure the successful achievement of Monarch HealthCare's vision. Primary Responsibilities:Work in close collaboration with the Disease Management department to provide telephonic disease state management for patients with the following chronic disease state(s): congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), Asthma, DiabetesAssist the Director of Pharmacy in planning, and implementing physician education programs related to new treatment modalities for various disease states, HEDIS/P4P, CMS Star Ratings, Medicare Part B and & D drugs, and pharmacy utilization management best practicesAvailable and acts as a clinical/pharmacy resource for Clinical Service's case managers including identifying high volume/high cost pharmacy utilizing members and assisting with the development of case management planningCollaborates with High Risk Touch Teams to ensure effective medication reconciliation and patient education for high risk patient in order to decrease readmissions and avoidable emergency room visitsParticipates in the inpatient readmission rounds and interdisciplinary (IDT) case management meetingsMaintains a working knowledge of changes in pharmacy practice, including pharmacy laws and drugs, and applies this knowledge in the performance of daily activitiesIdentifies opportunities for improvement and provides additional orientation, training and skill development to internal staff and external customers on an ongoing basisParticipates in the Clinical Services Medical Management Team and collaborates with the Pharmacy Dept. in all strategic planningCounsels patients and their care givers on safe and appropriate use of drugs
Required Qualifications: PharmD - current and unrestricted Pharmacist License for the State of CA Must have one of the following: Completion of a one (1) year Ambulatory / Clinical pharmacy residency program; OR Minimum 2 years of Clinical Experience with IPAs, medical groups or direct patient contact Experience with Medicare Part B and Part D Strong computer skills: MS Office- Word/Excel/Outlook Must be able to travel in the field 25% of the time, within Orange County (Dana Point to Long Beach would be the territory) Preferred Qualifications:MTM (Medication Therapy Management) Certification Current Geriatric Licensure or in process of obtaining: CGP (Certified Geriatric Pharmacist) BCPS - Board Certified Pharmacotherapy Specialist HEDIS and CMS star rating experience Bilingual in Spanish Previous experience with heart failure or diabetes What's the first thing that happens when one of the most successful physician groups in Orange County comes together with a global leader in health care?
