Rutgers New Jersey Medical School is seeking a licensed Social Worker I for the Pediatric Department. The Social Worker I is responsible for providing supportive social work case management services to the families of pediatric patients who are receiving assistance from H.E.A.L. Collaborative (Health, Education, Advocacy and Law), a medical-legal-social work partnership, while working collaboratively with members of the interdisciplinary team (e.g. healthcare and legal professionals and students) and the community to address systemic health issues.
Assesses the needs of pediatric patients and their families referred to H.E.A.L. Collaborative for assistance.
Develops and implements social work case management intervention plan(s) in collaboration with interdisciplinary partners and pediatric patients and their families.
Advocates with and on behalf of pediatric patients and their families who are clients of H.E.A.L. Collaborative to obtain needed public benefits, social services and other community resources.
Develops and maintains positive working relationships with interdisciplinary partners, healthcare and legal staff, local organizations and community groups to collaborate on health-related projects and initiatives.
Creates informational and educational materials and provides trainings to healthcare and legal staff and students as well as community members.
Maintains detailed notes and records regarding all work performed and prepares reports, as needed.
Preserves and protects patient/client confidentiality as part of the H.E.A.L. legal team.
Participates in IRB-approved research projects and assists with grant writing and reporting.
Supervises graduate level social work students as they complete their field placements in H.E.A.L. Collaborative.
Understands and adheres to Rutgers' compliance standards as they appear in RBHS's Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy.
Performs other related duties, as assigned.
Minimum Education and Experience:
Graduate from an accredited School of Social Work with a Master's Degree in Social Work.
Active license to practice social work in the State of New Jersey as a Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW).
At least three years of social work case management practice experience in health/mental, legal services, school, or other case management environment.
Required Knowledge, Skills, and Abilities:
Current in all continuing education requirements for licensure.
Excellent interpersonal and communication skills, collaboration skills, writing skills, public speaking skills, and ability to teach/mentor.
Bilingual Spanish/English highly desirable.
Physical Demands and Work Environment:
Must be able to lift or exert force up to twenty-five (25) pounds.
May need to accompany clients to public benefits offices, schools and other community agencies and, at times, meet with clients in their homes.
Time will need to be divided between office sites at Rutgers-NJMS and Rutgers Law School.
