Central Peninsula Care Transitions is part of a commitment to meet the needs of our community members who suffer from addiction. CPH Care Transitions is a new six-bed facility that focuses on withdrawal management for individuals needing inpatient, residential, and ambulatory withdrawal support. As a service of a Joint Commission accredited, Planetree designated hospital, we focus on high quality care and patient-centered services. Patients at CPH Care Transitions are provided the opportunity to step into a new life through access to a full continuum of treatment services. All of our addiction treatment programs focus on providing evidence-based treatment with a commitment to finding services that meet the needs of all our community members.
As a member of the CPH Care Transitions team, the Care Navigator is primarily responsible for the delivery of client care coordination. The Care Navigator partners with clients, families, medical, behavioral health and community providers across and between care settings ensuring coordination of services and seamless transition of care. This position is responsible for facilitating holistic, coordinated, individualized, client and family-centered care to improve and sustain quality of life.
Master's degree in mental health field (psychology, social work, or counseling)
Three to five years of addiction treatment experience
State of Alaska license as a mental health worker (psychologist, social worker, professional counselor) at time of hire.
Prefer experience in case management or utilization management.
