Responsible for case management of mental health services to about 100 clients per year to be conducted at homes and schools (as needed) according to assignment, within agency objectives, policies, and program.
Provides individual and group mental health counseling services for youth and families.
Develops diagnostic assessments and treatment plans with clients to ensure healthy pregnancy.
Maintains participant family records as required by the agency and program standards.
Compiles and enters statistical data as required by the Program and agency.
Provides data and information that will assist in program reporting.
Collaborates with personnel from relevant agencies, schools, communities toward implementing services.
Manages all agency expectations for case recording, statistical and other routine forms with accuracy, efficiency, and punctuality.
Attends and participates actively in meetings for clinical and/or full staff.
Participates in on-going training and staff development provided by Program Coordinator, Program Director, Director of Mental Health, other consultants, workshops and conferences.
