This position reports to the VP of Care Improvement. This is a full time position and will be responsible for directing the Thoughtful Life Conversation (known as End of Life Care) grant implementation process.
Works with an interdisciplinary group of healthcare professionals and community leaders across the state to plan, implement and evaluate advance care planning professional education and community outreach activities. Develops project plans and monitors progress for the Thoughtful Life Conversations grant. Position will coordinate meetings with key stakeholders and works in collaboration with other agencies/departments as appropriate. Manages day to day operation of assigned projects/activities related to Thoughtful Life Conversations. This position is grant funded for four years.
Bachelor’s Degree in a healthcare related field such as public health, social work, human services
Master’s Degree, experience in project management, social services, community outreach, education, palliative care or hospice.
Other: Position does require travel. Candidate must possess a valid Arizona driver’s license, current auto insurance, daily access to a reliable vehicle.
