Through the Guest Assistance Program (GAP), social workers serve as ambassadors/liaisons to the entire Social Work Department and to patients and families in general. GAP social workers provide support by assisting with information and referral, pharmacy and limited financial assistance, transportation, lodging, community resources, wig bank and other tangible needs to facilitate continuity of patient care, as well as access to care. This position is located in the Patient Assistance Center (PAC) of the Comprehensive Cancer Center and will provide support specifically to that patient population and staff.
This position requires brief psychosocial assessment skills to evaluate eligibility for resources, as well as ability to evaluate needs for referral and appropriate intervention for patients and families. This position demands the ability to work well with a diverse population in need of a variety of support and services. It frequently serves as an information resource and entry point to aid patients in managing new financial reality often created by severe illness. Work will be completed with multidisciplinary clinic staff to identify and optimize family information, education and patient assistance opportunities. Frequent collaboration with related programs such as Billing, Patient Financial Counselors, Pharmacy, Patient Lay Navigators is also required
Bachelor's degree in social work from a school accredited by the Council of Social Work Education (CSWE) is necessary. Must have LLBSW or LBSW license.
Excellent interpersonal and written communication skills are critical for success in this environment. Word, Access, Excel and Epic are utilized on a daily basis. Creating an exceptional patient/family experience is our highest commitment, and this role is expected to practice principles of patient and family centered care.
Knowledge of local and statewide community resources is desirable, as well as experience navigating complex systems. Previous experience in social work and/or healthcare or social service agencies will be particularly helpful for success in this role. Advanced critical thinking, assessment and problem-solving skills are crucial components of this role, as well as advanced interpersonal skill and the ability to establish patient relationships quickly.
Work Schedule is Monday through Friday, 8 hour daily shifts.
