Position Summary: We have an exciting opportunity to join our team as a Care Coordination Educator.
In this role, the successful candidate The Care Coordination Educator will be responsible for designing, implementing and evaluating educational programs using appropriate theoretical frameworks. With guidance from the Director of Education and Performance effectiveness this position ensures that all educational programming reflects current, evidence-based practices and incorporates principles of adult learning. This position will be responsible for the provision of care management orientation and continuing training program based on assessed needs. This position acts as an educator, internal consultant, facilitator, role model and change agent.
Identifies or develops department-wide or individual learning initiatives that cost-effectively focus employee development toward achievement of organizational as well as individual professional and career goals.
Develops cross continuum orientation and training programs for care management staff.
Conducts, documents, and communicates thoughtful analyses and monitoring of training needs.
Creates and monitors project plans to design and deliver world-class learning programs leveraging technology where appropriate to extend access, gain efficiency and scalability.
Creates standardized performance and evaluation measures across care management teams.
Develop relationships with the interdisciplinary team members, not limited to, providers, nurses, social workers, and rehabilitation teams to enhance learning opportunities for care management department.
Articulates well organized, compelling, appropriate, and innovative recommendations for learning solutions.
Implements quality and performance initiatives for the care management department based on ongoing analysis of training outcomes and feedback from staff and interdisciplinary team.
Conducts training needs assessment, recommending and implementing programs to support key skill areas.
Leads the implementation of learning solutions by partnering with subject matter experts and stakeholders
Functions as a subject matter expert regarding the design and development of innovative learning experiences at an enterprise level.
Remains current with the industry as it pertains to learning and development.
Performs other duties as assigned.
Minimum Qualifications: To qualify you must have a Masters degree in a relevant discipline required. Registered Nurse (RN) PREFERRED 10+ years of clinical experience 5+ years of experience in the area of program development Knowledge of adult learning principles and curriculum design Experience with the design and development of Instructor-led training, eLearning, social learning, and other innovative learning strategies Proven ability to adapt in a changing environment and to develop and implement effective training strategies, solutions and tools. Analytical skills with ability to draw conclusions from data Working knowledge of evaluation methods and principles of statistics. Strong project management and organization skills Experience curating content.
Required Licenses: Registered Nurse License-NYS
Preferred Qualifications: Doctorate in education
