Responsible for planning, directing, and evaluating the clinical services provided by the Nursing Department. Work is performed in coordination with the Director of Nursing (DON). May perform the following duties personally or through subordinate personnel.
Clinical Coordination: Participates in planning, directing, and implementation of clinical nursing services. Monitors workflow of nursing units to ensure work is performed timely and accurately. Recommends changes in clinical policy/procedures. Assists in assuring facility compliance with regulations. Ensures accuracy of physician orders. Assists in MDS process as needed. Audits clinical performance to identify opportunities for improvement or corrections needed: Admission process, Medicare compliance, documentation requirements, etc. Acts as a resource for nursing staff regarding policy and procedure and provides guidance as needed. Creates, updates, and maintains records in regard to facility psychotropic use. Performs AIMS testing for residents on antipsychotic or other medications with TD side effects. Develops behavioral care plan as needed for residents with psychotropic use. Participates in Wound Team. Performs weekly Norton Plus skin risk assessments as indicated. Ensures that annual resident TB skin testing is completed or CXR if indicated.
Supervision of Restorative Aides/Unit Coordinators/Level 2 CNAs: Interviews, hires, coordinates orientation, coaches, and completes performance evaluations for restorative aides/unit coordinators. Routinely seeks feedback from staff to identify opportunities for workplace improvement. Participates in restorative program supervision as required. Reviews physician order entry by unit coordinators and provides feedback routinely. Coordinates Level 2 CNA program and works with CNA IIs to develop plan to fulfill qualifying awards. Monitors mentorship program for improvement opportunities/training needs. Acts as LEAP trainer and coordinator.
Nursing Administration: Assists DON in managerial duties. Performs employee/volunteer health duties including immunizations, TB skin testing, etc. Participates in Quality Assurance activities including data collection, data analysis, and planning improvement actions for Infection Control, Safety and other. Participates in staff development for licensed and non-licensed staff. Shares on call responsibilities with DON. Prepares for annual survey by updating roster matrix and condition report as facility enters survey window.
Uses EPIC and other technology to enhance communication and coordination of services to residents. Investigates, plans, and implements improvements in ECS to achieve greater efficiency, effectiveness, and safety. Keeps access code for computer confidential at all times. Does not share access code with others and does not use other's access codes.
Safety: Complies with safety policies/procedures to ensure safe practices for residents, staff, self and others. Ensures compliance with: gait belt use for all manually assisted transfers, locomotion/transfer/lifting devices, resident positioning, and always uses mechanical lift to lift resident from floor. Uses good body mechanics while on duty. Identifies wandering residents and responds to door alarms. Uses and disposes of sharps according to safety standards. Reports safety incidents, concerns, or hazards to DON or other responsible parties as indicated. Takes corrective action for safety issues.
Infection Control: Complies with infection control standards during tour of duty including hand hygiene, appropriate use of personal protective equipment, and isolation precautions. Understands to use handwashing with soap and water when dealing with gastrointestinal illnesses. Maintains residents in isolation when indicated. Participates in mandatory vaccination programs. Does not work when contagious.
Privacy/Confidentiality: Maintains privacy and confidentiality for residents at all times including resident information in any format: paper, electronic, video, photographic, and conversation.
Associates Degree in Nursing required
Licensure by the State of Wisconsin as a Registered Nurse required
CPR certification required
Ability to communicate professionally with coworkers and residents
Ability to deal with time pressures and stress
Bachelor's Degree in Nursing preferred
Three to five years of previous nursing experience, especially in a geriatric setting, is preferred, along with previous supervisory experience
