This role will also travel to Reno, NV when needed*** Primary Responsibilities:Case manage Medicaid high utilizing members Assess members needs to maintain stability in the community Participate in members support plan accessing input of providers delivering care to member Arrange outpatient provider appointments, arrange transportation for member and participate in outpatient appointments with member Resolve barriers related to members care Perform administrative duties related to a members care Arrange for services as needed Develop positive therapeutic alliances with all levels of mental health providers and the member Assist with hospital discharge plan and achieve the goals set forth during inpatient treatment Support mental health services and integrated substance abuse treatment, supporting medication management, symptoms management, rehabilitation, crisis stabilization and psychosocial education on an outpatient basis Develop plans and supportive services for the individual to achieve stability and adaptive functioning to become independent Will be expected to manage case managers in Southern and Northern NV; team of 15
Required Qualifications:Undergraduate degree or equivalent experience LCSW, LPC, LMFT, or RN with an unrestricted, active applicable NV state license. 1 year+ Case management experience 2 years+ Behavioral / mental / substance abuse background Proficient in Word, Excel, PPT, etc.Preferred Qualifications:Manages and is accountable for professional employees and / or supervisors. Knowledge of case management concepts, standards and alternative delivery systems. Knowledge of state and federal regulatory requirements, as well as certification requirements Knowledge of Medicaid insurance Strong interpersonal, organization, written and oral communication skills Computer literate and database familiarity Ability to manage conflict with strong problem resolution skills Ability to build therapeutic relationshipsCareers with Optum.
