All the while, you'll be building meaningful relationships with the patients, their families, and the health care providers who are responsible for their care *This is a field - based position that requires daily travel to our members place of residence" Primary Responsibilities:Provide hands-on, post-hospitalization patient care services to members in their place of residence Conducts a physical assessment, performs a medication reconciliation and assists with referrals to Specialist as needed Will work with members who are at high-risk for readmission to a hospital Communicate findings in your assessment that will be used to inform the Case Manager, Disease Management or Palliative Care team of potential gaps in care Educate members on topics such as disease process, medication, and compliance
