JOB LOCATION The Ambulatory Care Pharmacist position is located in Indianapolis, Indiana.
The Ambulatory Care Pharmacist works closely with physicians and health care teams, while providing direct patient
Using a collaborative drug therapy agreement, the Pharmacist will meet with patients individually to provide extensive drug and disease state education with a particular focus on patients at high risk for hospitalization or poor health outcomes
Will optimize medication regimens for patients with a variety of conditions, such as diabetes, hypertension, dyslipidemia, asthma, COPD, tobacco dependence, and osteoporosis
Will work closely with patients to overcome medication adherence issues
Uses various strategies to lower patient medication expenses
Will serve as a drug information resource for the Physicians and nursing staff, answering medication therapy questions

Completion of a PGY1 residency (or equivalent) is required
Completion of PGY2 residency with an ambulatory care focus is strongly preferred
Education Graduate of accredited college of pharmacy required.
Valid Indiana Pharmacist license.
Board Certification is expected within 2 years of working in this position.
