Position Summary: We have an exciting opportunity to join our team as a Transplant Nurse Navigator.
In this role, the successful candidate The Transplant Nurse Navigator is an important member of the multidisciplinary transplant team. The Navigator assists patients and community physicians to ensure rapid and coordinated evaluation for transplantation. Specifically, the Navigator is responsible for: planning, organizing and conducting transplant education with community partners including physicians, nurses, practice managers, dialysis unit personnel, and other providers that participate in the care of patients with end stage organ failure; reviewing the status of patients who are referred for transplant to ensure that they move through the transplant evaluation and organ wait-listing process in a timely manner; assisting in creating multi-media products (such as educational videos, website content, and patient brochures) that further the outreach and education mission of the Transplant Institute; providing education to patients, families, and potential living donors; representing the Transplant Institute at local and regional conferences and events; communicating effectively with referring physicians about the status of mutual patients; and participating in a team that provides 24 hour on call coverage of patient calls.
Leads and coordinates efforts with clinical and administrative professionals to present a unified image of the Transplant Institute. o Maintains a strong web presence; website is reviewed and updated at least every quarter. o Collaborates with clinical and administrative staff to identify outreach and education opportunities as demonstrated by periodic attendance at organ specific team meetings and proactive interaction with medical and surgical directors. o Receives positive feedback from staff and external clients as demonstrated by comments made by patients and/or physicians.
Plans, organizes and conducts transplant education with community partners, including physicians, nurses, practice managers, and other community providers that participate in the care of patients with end stage organ failure.
o Conducts a minimum of 12 outreach activities per month; of which at least 3 are physician to physician meetings with the remainder made up of physician office (or dialysis unit) visits conducted by the nurse navigator. o Oversees NYU sponsored patient and community education events including planning, advertising, speaker coordination, and conference logistics, achieving at least 80% of expected attendance at each event. o Implements strategies identified by leadership to enhance communication and provide education to existing and potential referring providers and patients of the transplant programs.
Reviews status of new referrals to ensure patients move through the transplant evaluation/listing process in a timely manner. o Reviews status of new referrals monthly. o Identifies patients who have stalled in the transplant process and collaborates with transplant staff to resolve. o Identifies and reports patterns of inefficiency and partners with clinical and administrative staff to implement process improvements needed to alleviate future delays.
Assists in creating multi-media products that further the outreach and education mission of the Transplant Institute. o Manages the inventory of the Transplant Institute educational and marketing materials. o Collaborates with NYU Marketing and Communications staff. o Recommends Facebook content for the NYU page. o Partners with clinical and administrative transplant staff to write updates and design patient educational materials, including patient handbooks and ensure that publications are continually reviewed, re-written and kept current.
Provides nursing support as needed to current and future off-site clinics as services expand and develop to meet the needs of the transplant programs. o Conducts pre-transplant evaluations of patients as indicated with multidisciplinary team members. o Identifies opportunities for increasing referral volume at off-site locations.
Tracks and reports outreach activities and maintains, enhances and develops new relationships with community partners. o Completes quarterly activity report that includes targeted, completed, and upcoming outreach initiatives. o Provides market intelligence gained during community activities. o Provides timely and accurate communication with community partners. o Partners with leadership to track and identify community physician referral patterns following outreach activities.

Minimum Qualifications: To qualify you must have a Licensure: Current registration as a Professional Nurse in New York State
Education: Bachelors Degree in Nursing
Experience: At least two years of clinical nursing practice, ability to apply nursing process toward achievement of specific outcomes in an organized fashion.
Competencies: Self-directed worker, able to work independently; excellent interpersonal, communication and organizational skills; creative problem solving and critical thinking ability, understanding of complex antibody profiles..
Required Licenses: Registered Nurse License-NYS
Preferred Qualifications: Previous experience in transplantation, VAD, ECMO, public health education, heart disease, lung disease, renal disease, dialysis, or liver disease is preferred.
