The Navigation Manager will conduct staff trainings, develop and distribute policies and procedures and standard operating procedures, and serve as a back-up for Navigators in clinical delivery sites (e.g. hospitals and federally qualified health centers). Primary Responsibilities:Conduct Navigator trainingsDevelop and distribute policies and procedures and standard operating proceduresServe as a back-up for Navigators in clinical delivery sites (e.g., hospitals and federally qualified health centers or FQHCs) Act as a representative of myConnections to both consumers and community partnersRefer and help navigate and assist consumers with accessing appropriate social services providers based on intake information, consumer socioeconomic status, and consumer's schedule Understand the target population through experience by integration into the local community Recognize local social service providers that connect with target populations through continued community relations and outreach Function as an advocate for consumers Collaborate with other onsite Navigators to ensure coordinated action plans are implemented for each consumerEnsure intakes and referrals are documented in a timely mannerContribute to community education and outreach efforts The Navigation Administrator must Reside in Honolulu and be familiar with the local community
Required Qualifications: High School Diploma / GED or higher level of education2+ years working in a highly matrix environmentGeneral executive presence / presentation skills2+ years of experience with Medicaid and Medicare populations4+ years of social services and social determinants of health (e.g., Housing, Transportation, Food Security, Utilities, etc.) experiencePreferred Qualifications: Undergraduate degree or higher level of education in Social Work, Behavioral Health, Public Policy, Public Health and/or Health Administration 5+ years of leadership experience (Supervisor, Manager, and/or Team Lead)Experience working with Native Hawaiians, Pacific Islanders, immigrants from Micronesia, and Waianae Coast residents and who are bilingual (Hawaiian, Samoan, Tahitian, Tongan, Maori, Tagalog, and/or Fijian) 5 + years leadership experience in Hospitals, Behavior Health Clinics, Community Health Organizations 5+ years supervisory experience and managing in a virtual team setting Healthcare industry and For profit experienceCareers at UnitedHealthcare Community & State.
