This is the place to do your life's best work.(sm) Pediatric Case Managers are responsible for coordinating and managing medical and behavioral health services delivered to youth who are in foster care. The service coordinators have experience working within the foster care system and also have knowledge of services utilized by foster care youth, especially behavioral health services. The service coordinators serve as the primary point of contact for the youth, their family, the state child welfare contractor, foster parents or group home placements and network providers. In this role, you will work in the field and coordinate care for youth who are in foster care. Your experience working with foster care youth and with behavioral health services will be essential in advocating for the best possible care available. At times, your patience may be challenged. This territory will cover St. Louis and Saint Louis City County, MO.** Primary Responsibilities: Leverage experience working with the Department of Social Services, Children's and MO HealthNet Divisions and have knowledge of services used by children and youth, especially behavioral health services Serve as the primary point of contact for the member, Social Service worker, guardian/caregivers, network and other providers Coordinate medical and behavioral health services delivered to children and youth who are in foster care, receiving foster care or an adoption subsidy, or are in or out of home placement including ensuring that the child receives all required examinations and health care visits within the time specified by the state agency and based upon the child's needs Will serve as the primary contact for staff, health care providers and caregivers for children in the custody of the Saint Louis County and Saint Louis City County Offices of the Missouri Children's Division This designee will participate in and attend medical oversight meetings and cooperate with the Medical Care Management Agency to secure medical histories and provide medical records Conduct evaluation of member needs in collaboration with the youth, Social Service worker, caregiver/guardian and develop a care plan to address member needs Review and revise plan of care as needed based upon the needs of the member Work collaboratively with service providers, families and Social Service worker to identify and access resources to meet the needs of members and support stable placements and permanency planning Work with Social Service worker and network providers to ensure timely access to services following discharge from hospitalization or residential treatment In collaboration with Social Service worker, provide outreach and coaching to members, families and foster care placement to encourage and promote treatment and service adherence Respond promptly to emerging issues that impact services to the child/youth Support transition age youth and eligible members to age 26 with access to care and care coordination needs
Required Qualifications: RN License, must be current and unrestricted in state of Missouri 3+ years recent experience in hospital, acute care, home health, hospice, case management, foster care system and/or social service coordination High Speed Internet access in home Strong computer proficiency (Microsoft Word, Outlook, and Internet) Willing and able to travel throughout surrounding area on regular basisPreferred Qualifications: Foster Care experience Experience with the Department of Social Services, Children's and MO HealthNet Divisions Home care and/or case Management experience Certification in Case Management (CCM) Experience with Medicaid and/or Medicare Managed Care experience Experience working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needs Bilingual skillsCareers at UnitedHealthcare Community & State.
